Who Can Benefit From Non-Surgical Rhinoplasty?


Non-surgical rhinoplasty, also known as a non-surgical nose job, injection rhinoplasty, or the “15-minute nose job”, can be a lower-risk, lower-cost alternative (with less pain and less downtime as an added bonus) for those who may have otherwise considered surgical rhinoplasty. For those who can’t afford the expense or downtime associated with surgery, or for those who are simply afraid to actually go under the knife, it can be quite a relief to hear that there is such an option available.
While non-surgical rhinoplasty cannot reduce the size of a large nose, the creation of symmetry or smoothing out of contour irregularities can actually make the nose appear smaller on the face. Furthermore, the non-surgical nose job can provide a non-invasive alternative for revision surgery for patients dissatisfied with the results of surgical rhinoplasty.
Non-surgical rhinoplasty can be useful to:
·         Reduce the appearance of a “bump” by filling out the surrounding tissue
·         Reduce or eliminate unsightly depressions, dents, or grooves
·         Improve symmetry
·         Create a lifting effect on a drooping nasal tip
·         Raise and define a flat bridge or de-emphasize a rounded tip (popular in Asian and African-American populations)

How Non-Surgical Rhinoplasty Is Done

Tiny amounts of an injectable dermal filler (such as Radiesse, Artefill, Juvederm, or Silicone) are injected into areas that need to be filled out to achieve a smoother, more symmetrical appearance. The filler is injected in miniscule amounts using what is called a microdroplet technique.
This procedure is not exactly a quick fix, as the end result is only achieved after a series of several treatments, with at least 4-6 weeks in between sessions. This gradual approach is necessary because of the way these fillers work: After injection, your body’s defenses begin working to “wall off” each microdroplet of the filler material, forming a capsule of tissue that holds it in position. Over a period of time, the natural tissue built up around the filler creates more volume in the area, so the result cannot be accurately judged at the end of each injection session.
Only after the prescribed amount of time is it possible for the doctor and patient to judge if and where more injections are needed, as well as how much is needed. This allows to doctor and patient to judiciously decide when enough is enough.

Is It Safe?

Non-surgical rhinoplasty does not require general anesthesia or IV sedation. Only a local anesthetic is used, sometimes in conjunction with an oral sedative to relax the patient. Because of this and the fact that there is no actual cutting involved, this method carries less risk than a surgical rhinoplasty procedure. There are, however, some risks, which include:
·         Hyper-sensitivity reactions
·         Infection
·         “Lumpy”, asymmetrical, or otherwise unsatisfactory aesthetic results
·         Nasal skin damage
There are also risks associated specifically with silicone injections, such as migration (movement of the silicone to another part of the face or body), and the formation ofgranulomas. However, these problems have historically been mostly associated with the injection of larger quantities of silicone by unqualified injectors.
It is believed that the microdroplet method significantly reduces the risk of these complications. Furthermore, because the nose is a virtually static (having limited movement) area of the face, the possibility of silicone migration is much less. In fact, Beverly Hills cosmetic surgeon Robert Kotler, MD, who has been performing non-surgical rhinoplasty for over thirty years, says he has never seen a significant complication in his own practice with this use of silicone.

What Cannot Be Accomplished With Non-Surgical Rhinoplasty?

If you want a smaller nose, a thinner bridge, a large bump removed, or a more narrow tip, this procedure will not accomplish your goals. Non-surgical rhinoplasty cannot improve breathing problems or correct internal defects such as a deviated septum. Nor can it (or any other procedure) give you a “perfect”, completely symmetrical nose. As always, the goal of cosmetic surgery is improvement, not perfection.

The Downside

Injections with Juvederm, Radiesse, or most other fillers are temporary. To maintain your results, you will have to come back again and again for injections. This can be inconvenient and could become quite expensive over time.
On the other hand, injections of silicone or Artefill are forever. You can’t change your mind and have it removed, except through painful and disfiguring surgical excision. Because of this, selection of a highly-qualified surgeon is exceedingly important.
People with autoimmune disorders should not undergo injections with silicone or other permanent fillers, and those concerned about hyper-sensitivity reactions should ask their doctors to perform a skin test before going ahead with the procedure.
If you are considering surgical rhinoplasty at any point in the future, you should not have injection rhinoplasty with a permanent filler like Artefill or silicone.

WHAT IS THE STEM CELL FACELIFT?


The stem cell facelift is a complete facial rejuvenation procedure that does not involve surgery. The procedure avoids any incisions and helps in the lifting of the skin itself. The main function is to restore both the youthful contour and shape of the face. Another goal is to improve the skin quality and color irregularities caused by both the aging process and exposure to the sun and environment. Patients who have undergone this procedure frequently avoid or markedly reduce the need for other maintenance procedures like Botulinum toxin and cosmetic filler injections.


The function of the procedure is based on the regenerative capacity of a group of cells called ADULT STEM CELLS. Of the hundred trillion cells that make up the human body, stem cells are the most important category. They are among the few cells in the body that are specialized and have the potential to become a particular cell type and replace worn and damage cells.


A large number of stem cells exist in around adipose or fat tissue in the stroma. When stem cells are isolated and then transplanted either with or without fat into parts of the body, studies have shown that they grow and survive in these new areas helping to form new tissue at the transplanted site. By using the regenerative properties of stem cells either alone or along with transplanted fat, a marked improvement of survival of these tissues occurs.


What makes the stem cell facelift unique and different from other fat transplant procedures is the fact that the autologous (from self) isolated stem cells can be placed into areas that have had transplanted fat, and/or autologous Growth Factors (PRP - Platelet Rich Plasma), and/or Hyaluronic acid. This can help the stem cells and adipose tissues survive and grow into the needed tissue to restore lost fat, enhancing the volume of the face, and skin elements which would improve both the quality and texture of the Skin.




WHAT IS THE PROCEDURE AND HOW IS IT DIFFERENT FROM OTHER "STEM CELL" FACELIFTS?


The stem cell procedure is performed under local tumescent anesthesia. After injection of the tumescent solution to the desired areas (lower abdomen, inner thighs, etc.), a fat sample or lipoaspirate (liposuction) is used to gather the required cells.


During the procedure and after removing a desired amount of fat lipoaspirate (adipose), the fat is then processed like a regular stem cell procedure. Most clinics claiming a stem cell procedure do not actually separate-out the stem cells from the fat stromal compartment. This is essentially leaves the stem cells "locked-within" the surrounding collagen sheath, thus limiting or eliminating their potential to grow. We find it absolutely ludicrous to promote a "stem cell" procedure that is essentially a fat transfer with or without growth factors. Here at the Stem Cell Rejuvenation Center, we only provides the real "extracted and isolated" stem cells for the stem cell facelift procedure. The entire high-quality procedure takes at least 6 hours.


Autologous Growth factors can be used along with adipose fat cells (not the ones used to extract and isolate out the stem cells), Hyaluronic acid, and the isolated stem cells themselves.

Botox Surgery Procedure


Botulinum toxin, widely known as BOTOX, is a purified toxin produced by bacteria that causes weakening and temporarily relaxes muscles.  It has been used for over 2 decades in the treatment of muscular problems of the eyes with great success. Just recently, the Food and Drug Administration (FDA) has granted approval to administer BOTOX for the temporary improvement of facial expression lines in adult men and women. BOTOX is one of the most popular cosmetic treatments today. This procedure can be done on a lunch break and requires no downtime or recovery time.

Small amounts of BOTOX are injected with a fine needle into the muscle that creates a wrinkle. There are three areas usually treated:  Frown lines, or the lines in between your eyebrows (elevens), crow’s feet (the fine lines or rhytids outside the eyes), and horizontal forehead lines, are the areas most commonly injected. As the muscle weakens, it relaxes, causing the wrinkles in the skin to gradually soften and often disappear. 

Side effects of Botox Surgery
Some people have reported little headaches after treatments. A small bruise at the site of injection is the most common side effect. When BOTOX is injected into the forehead, it can result in brow-muscle weakening causing a temporary lopsided appearance. This is extremely rare and usually goes away by itself.

The effects of BOTOX usually last three to five months, at which time another injection is needed. After several treatments the effects of BOTOX may begin to last longer, sometimes up to eight months.  Overall, the vast majorities of patients love this quick-fix therapy, and return for a BOTOX boost as soon as the wrinkles begin to reappear!

Recovery from Botox Treatments
After most botox treatments, the patient can instantly return to their normal activities with a few common-sense limitations. Following the post-procedure guidelines, can dramatically reduce the length and intensity of your recovery period. Naturally, you should avoid heavy physical exercise, sweating unnecessarily, applying cosmetics, or laying down flat for the first 4 hours after the injections. It is also advisable to avoid rubbing or massaging any of the areas of the face where the Botox was injected for at least 24 hours after the non-surgical cosmetic facial procedure.

Dr. Berman's Cosmetic, Plastic, & Reconstructive Surgery Experience

The 2011 New Year has just arrived.  I wanted to write an article about cosmetic surgery: the latest and greatest of what we do to keep people looking and feeling younger. Like most Floridians, I moved down here for the warm weather, particularly the permanent golf season.   In the past year, I developed a passion for road cycling to exercise my heart and regain physical health.

While rummaging through my desk I found three old photos of me.    The oldest was from seven years ago.   I had just lost65 pounds on an Atkins Diet.   I went from 220 to 165 pounds.  The second picture was from 2 ½ years ago on vacation in Greece when I weighed about 185-190 pounds.  Check out my belly, that bulge revealed the pleasures of the “good life.”  Both of my daughters had just graduated – Brittany from high school, Alexandra from college.  My wife, who I adore, always looks beautiful, young and thin. She is always eating but in tiny morsels like a bird and rarely gains an ounce.   I know she could easily fit into her wedding gown from 26 years ago.  She still wears a size zero.   Barbara is much luckier than most. I, like most of us, regularly gain and lose weight, and force myself to stay “within the limits.” It is all genetics. The third picture of me was taken within the last 2 months. I still weigh 185 pounds. What a positive change!

How did I make that change? About 18 months ago, health issues forced me to take a good look at myself and my mortality.  I knew I needed to start exercising to stay healthy and keep my heart in the best shape possible.  It’s easy to forget that we all need exercise.  So what to do?   Run? How could I start running at 50? Everyone I know who runs develop badly back, hip, knee, foot, or ankle problems. Swim? Swimming is great yet it forces you to join a gym and is a very antisocial sport. Row?  Rowing a crew shell is another wonderful cardiovascular exercise, yet access to a crew shell is difficult. Besides, would you want to row in the intercoastal? Join a gym and become a gym rat was another option, but I don’t really like those machines and always feel like a hamster when I get on a treadmill. 

My good friend, Phil, suggested that I buy a road bike.  He reminded me that there was no impact on hurt knees, ankles, hips or back.  The only drawback is the occasional motorist that can hit you with their car, so he told me “buy a rear view mirror for your helmet!”Camaraderie was automatic once I began riding with apeleton(until I began riding, I didn’t know that a peleton was a group of riders).   Cycling appealed to me because it is a social experience as well as a good cardiovascular work-out. 

So I bought a road bike.  Not just any road bike, but one with a carbon frame.   A cycling outfit, called a cycling kit; as well as a helmet, and a pair of gloves.  Lance Armstrong, here I come!  Hard to believe I would ever wear a spandex/lycraform fitting outfit in my always pudgy frame.   So slowly but surely, I rode better, faster, and longer.  Who would have ever thought that I would be riding 120-140 miles a week? Last October when I started, no way!  But now look at my picture from 1-2 months ago.   What did I learn?

I learned that getting fit is very important.   Fitness is not merely getting a better, healthier body, but a better healthier mind, too.  We can all agree that it is healthier to be thin rather than to be fat.  But too thin is also a problem.   After my 65 pound weight loss, I was too thin.  But now, with some physical fitness and meat on my bones, I have transformed from once pudgy and big-bellied to a fit, lean cycling machine!  I almost have a six pack!   My body’s shape has changed----I am leaner, more muscularly well defined but not really ripped.  My waist has decreased to a 32-33 and my neck size has decreased probably ½ inch.  Not only do I look the best I have looked in the past 25 years but I also feel that much younger - without plastic or cosmetic surgery!  Would cosmetic surgery have worked for me? Probably, since liposuction would have surely rid my torso and abdomen of unwanted fat.  But would I be healthier?

I have always battled my weight and waste line, and was chubby even as a kid.  I probably had a food problem because I had a very caring mother who only wanted the best for her overweight son: “Don’t have that roll,” or “Leave over the potatoes,” or “Don’t have seconds of the dessert.”   Like most of us with a weight problem, losing the weight is battle number one.   Battle number two is keeping it off.  But by comparing the picture from Greece with the picture of me now, looking and feeling good is about more than just a number on the scale.   It’s about the physical condition and fitness of our bodies.

Don’t misunderstand, cosmetic surgery is wonderful and I have helped many people with their cosmetic needs.   But for some people, finding a recreational activity that provides physical fitness can do the trick.  Cycling has rescued me – it has promoted a healthy lifestyle and given me a passion.

We should all be so lucky.  Passion gives meaning to our day to day experiences.  It creates excitement.   It makes the heart race.  I heard a Rabbi in a recent sermon say that it is important for us to LIVE, embrace and enjoy our lives, to have others say of us, “he/she really lives life to its fullest.”  It is hard to live life to its fullest without passion.  Participating in activities that we are passionate about causes us to be happier and better people by giving us a sense of self-worth and confidence.  It helps us conquer bad days and alleviate our daily toils and stresses. 

My advice: find a passion and pursue it.  It will make you feel better about yourself.  You will feel confident.  When you are confident, you will take the necessary steps to keep yourself healthy.   That seemingly hard “fitness program” or daily discipline will become as easy as brushing your teeth every morning. The benefits will become evident:  a stronger mind, a stronger body, a better you. 

Having said all this, I can’t escape from the fact that I am healthy, young 51.  Unfortunately, my bathroom mirror only reveals the true me---my girls are right, I need a neck lift!
